Open-enrollment guidelines and application
- Open enrollment for 2025-2026 is closed. For consideration, contact the principal.

Wait to complete the below steps until notification of acceptance of open-enrollment.

Begin completing enrollment forms through Final Forms.
Obtain necessary documents:
- Birth certificate
- Social security card
- Immunization records
- Custody paperwork or IEP records (when applicable)
Contact the appropriate school office to schedule an enrollment appointment:
- Elementary school (grades K-6): 419-963-3415, option #1
- High school (grades 7-12): 419-963-3415, option #2

Note that school building offices are closed mid-June through early August - please leave a voicemail and you will be contacted once our staff returns from summer vacation.
